To obtain the card balance or fee information, visit http://balance.WorldPay.us, or call 800.803.1222 and enter the card number.
 
This card can be used for purchases at participating locations of the Merchant named on the face of the card.(gcb.today#40B4E). The card is not redeemable for cash, and lost or stolen cards will not be replaced, except as required by law. Unless waived by the Merchant or otherwise prohibited by law: 1) after 12 months of inactivity, a service fee of $2.00 per month will be deducted from the card balance; and 2) the card and the funds underlying the card, expire after five years from date of last load of funds.
